In order to do so, we will ask that you send certain medical information to our office before your initial visit. We will request some or all of the following:
- Documentation pertaining to tests and procedures you have had
- General office forms
- Identification and insurance cards
- Pathology report and slides (breast tissue on glass)
- Radiology reports and images on a disc (in diacom format)
