When it comes to treating breast cancer you should never have to wait to receive the care you need. At The Blavatnik Center, we are committed to offering you world-class care as soon as you need it. In order to do so, we will ask that you send certain medical information to our office before your initial visit. We will request some or all of the following:

• Documentation pertaining to tests and procedures you have had
• General office forms
• Identification and insurance cards
• Pathology report and slides (breast tissue on glass)
• Radiology reports and images on a disc (in diacom format)
