Patient and Family Advisory Council
The Mount Sinai South Nassau Patient and Family Advisory Council fosters an environment where you and your family feel known and respected. We want you to partner in your own health care.
We want you and your family to have a positive, compassionate, and supported health care experience. To achieve this, our whole team works closely with you and your family. We aim to:
- Promote a positive relationship among Mount Sinai South Nassau, you, and your family
- Improve patient and family satisfaction
- Collaborate with Mount Sinai South Nassau leadership and offer suggestions to improve the quality of care
- Offer suggestions on programs, policies, teaching, and educational materials
- Advocate for our patients within the communities we serve
- Help educate current and future health care providers
Our Contributions
We have improved the processes and programs at Mount Sinai South Nassau by developing:
- Updated white boards in patient rooms offering detailed patient information
- SMART Discharge Journal folders, which we give to you when you arrive with the goal of enhancing communication with your care team
- “Guest packs” that include blankets, toothbrush, and other items for family members who stay overnight
- Rounding by volunteers to ensure that you have easy access to hospital information and services
- Detailed signage throughout the hospital to help you and your family find your way around
- “Sweet Dreams” carts containing blankets, playing cards, puzzle books, tea, and other items distributed to patients by the comfort squad
- “About Me” patient boards showing preferred language, nickname, names of pets, and other personal details
Interested in Joining Us?
We invite selected community members to join us from among recent patients and family members. We are especially interested in people who show:
- A commitment to quality health care, patient outcomes, and satisfaction
- The ability to support our patient and family-centered care values
- A desire to be part of decision-making processes and provide support for the Council’s recommendations
- Respect for patient confidentiality and protection of personal or confidential information
- Willingness to attend a monthly meeting
