Mount Sinai at Home

Mount Sinai at Home provides medical services for  patients in the comfort of their own homes. Some of these patients might otherwise need to be in the hospital, emergency department, doctor’s office, or a rehabilitation facility. A team of doctors, nurse practitioners (NPs), physician assistants (PAs), registered nurses (RNs), and other professionals treat and monitor your health during and following an illness or injury. We also provide primary and palliative care for complex patients who have difficulty accessing medical care, and provide home health care for eligible patients who need support between medical visits.
