That is the premise behind the Mount Sinai at Home award-winning Home Health Care agency operated by a partnership between Amedisys/Contessa Health and Mount Sinai. If you need expert medical care to help you or a loved one, a dedicated team of professionals will bring specialized care and the latest health care technology right into your home. You don’t have to be a patient at Mount Sinai to use Mount Sinai at Home’s services. The Mount Sinai at Home staff cares for infants, children, and adults seven days a week throughout Nassau County and parts of Queens and Suffolk Counties. A registered nurse is available after hours, weekends, and holidays for phone consultation and home visits, when necessary.

Mount Sinai at Home Experts

One of Mount Sinai at Home’s case managers will work closely with you and your physician to coordinate all aspects of your care at home, including visits from specialists and any medical equipment. Mount Sinai at Home’s caregivers include registered nurses, home health aides, and therapists. Mount Sinai at Home staff has training in specialized services, such as rehabilitation after joint replacement, wound care, vestibular rehabilitation, and fall prevention. Mount Sinai at Home patients and their loved ones also have access to a registered nurse who is on call around the clock to answer questions and help if you have an emergency or other urgent need.

Mount Sinai at Home can work directly with you and your family, your physician, other specialists and therapists, hospital or other health care facilities, and insurance companies to develop a personalized home care plan. This helps ensure you have the care you need to live comfortably and safely at home.

Mount Sinai at Home Caregivers include:

Registered nurses who specialize in home health care among various other areas of care. Your registered nurse will take your vitals, and monitor your blood pressure, temperature, and weight. They will administer IV medications, help you take your medications, assist you with pain management, and serve as an advocate for your care.

Home Health Aides assist you or your loved ones with bathing, dressing, eating, and other activities of daily living. 

Registered physical, speech, and occupational therapists, many of whom have completed advanced training in their specific areas of service.

Medical social workers are available to assist you and your family with the emotional and social aspects involved in medical care, including health care decisions and long-term planning.

Registered dietitians to guide you through nutrition counseling and medically prescribed diets.
