Prostate Cancer Screening and Diagnosis

Early detection gives you the best chance for successful prostate cancer treatment. At Mount Sinai, our specialists provide personalized prostate cancer screening and diagnostic testing based on your individual risk factors, helping identify cancer as early as possible.

The American Urological Association recommends that men age 45 and older talk with their doctor about the benefits and risks of prostate cancer screening. Some men, including those who are Black and/or have family history may benefit from screening starting between ages 40 and 45 based on their personal risk.

Who Should Consider Prostate Cancer Screening?

Your doctor will recommend a screening schedule based on factors including:

  • Age
  • Race
  • Family history of prostate cancer
  • Personal health history
  • Previous prostate-specific antigen (PSA) test results

If traveling to a medical center is difficult, Mount Sinai also offers prostate cancer screening through our Mobile Prostate Cancer Screening Unit.

Prostate Cancer Screening Tests

Our prostate cancer specialists may use one or more of the following tests to evaluate your prostate health.

Prostate-Specific Antigen Blood Test

A prostate-specific antigen (PSA) blood test can help detect prostate cancer early and assess your future risk. Men at average risk may consider a baseline PSA test between ages 45 and 50, while men at increased risk may benefit from starting between ages 40 and 45.An elevated PSA level does not necessarily mean you have prostate cancer, and no PSA level can guarantee that cancer is absent.

Rather than relying on PSA results alone, Mount Sinai physicians evaluate your:

  • Age
  • Race
  • Family history
  • Prostate size
  • Percentage of free PSA (PSA not bound to other proteins)

This comprehensive evaluation helps determine whether additional testing is needed.

Mount Sinai is the only New York State-approved laboratory for point-of-care PSA testing, allowing us to provide PSA test results in as little as 20 minutes. Having results available during your visit can help you and your care team discuss your results and determine appropriate next steps without waiting for standard laboratory processing.

Digital Rectal Exam

Used together with PSA testing, a digital rectal exam allows your doctor to feel the prostate for changes that may suggest prostate cancer or another prostate condition.

Prostate Biopsy

If additional evaluation is needed, your doctor may recommend a prostate biopsy.

During a biopsy, a small sample of prostate tissue is removed and examined under a microscope. A biopsy is the only test that can confirm a diagnosis of prostate cancer.

Advanced Imaging and MRI Fusion Biopsy

Mount Sinai uses advanced imaging technology to improve prostate cancer diagnosis.

One advanced approach is MRI fusion biopsy, which combines magnetic resonance imaging (MRI) with real-time ultrasound guidance to accurately identify, target, and sample suspicious areas of the prostate.

This technology helps improve diagnostic accuracy while reducing unnecessary biopsies.
