Mount Sinai Visiting Doctors Program

Since 1995, the Mount Sinai Visiting Doctors Program (MSVD) has been providing high-quality, patient-centered and compassionate care to adults who are unable to leave their homes. With a team of physicians, social workers, nurses, nurse practitioners, administrative support, and community health workers, MSVD works to improve the quality of life of homebound people, as well as their caregivers, offering the best possible services and support.

Today, serving as both a health care service and a teaching platform for medical students, residents, and fellows, the Mount Sinai Visiting Doctors Program stands as a flagship clinical initiative. It is one of the largest academic home-visit programs in the nation, with its clinicians making more than 6,000 home visits annually to more than 1,000 patients.

The Care We Provide

Throughout Manhattan, the Visiting Doctors Program delivers comprehensive primary as well as palliative care to an underserved and growing segment of the population—homebound adults. Our team also works closely with families and caregivers to offer comprehensive management, support, and advocacy. 

Our Program provides personalized, home-based primary and palliative care to people with complex and serious illnesses. We coordinate care with a range of health professionals in the hospital and home setting, working closely with home care, visiting nurse, community, and hospice agencies. We are available by phone 24 hours a day, seven days a week, and our health care providers will respond to requests for emergency visits and have language translation services available 24 hours a day. We also are able to connect to patients via telehealth options. 

The home-based services we provide include:

  • Primary and preventive health care
  • Complex symptom management and palliative care
  • Diagnosis and treatment of medical conditions
  • Prescription of medications, with an emphasis on safety and affordability
  • Support to family members and other caregivers
  • Referrals for rehabilitation, nursing, hospice, and other home-based services
  • Assistance with practical needs, including wheelchairs and other durable medical equipment

Interdisciplinary Team

The Visiting Doctors Program staff crosses a number of disciplines. The program has a dedicated team of social workers who assist homebound patients and their families with access to services and provide counseling and support in English and Spanish. We also have a team of nurses and advanced practice nurses who work closely with patients and families to ensure the highest quality care.
