Your Visit

At Mount Sinai Ambulatory Care, we want your visit to be as convenient and pleasant as possible. Here is some practical information we hope you will find helpful.

Each Ambulatory Care practice and specialty service has its own hours of operation. Most require that you make an appointment ahead of time. Check the individual practices and specialty services for more information.

Bring insurance coverage identification: Bring any insurance coverage identification (such as a Medicaid card) along with any other information your primary care doctor has instructed you to bring to your visit.

Fill out the Patient Registration Form in advance: You can save time by filling out the Patient Registration Form and other required forms and bringing them with you. (See Related Resources, on the right hand side of the page.)

Plan to arrive at least 20 minutes before your scheduled appointment: If you cannot keep your appointment, please call the practice or specialty service to reschedule.
