Mount Sinai Ambulatory Care wants your visit to be as convenient and pleasant as possible. Here is some practical information we hope you will find helpful.
- Hours of Operation. Each Ambulatory Care practice and specialty service has its own hours of operation and most require that you make an appointment in advance of coming in. Check the individual practices and specialty services listed at the left for more information.
- Bring insurance coverage identification. Bring any insurance coverage identification (such as a Medicaid card) along with any other information your primary care doctor has instructed you to bring to your visit.
- Fill out the Patient Registration Form in advance. You can save yourself time by filling out the Patient Registration Form and other required forms and bringing them with you. (See forms at right)
- Plan to arrive at least 20 minutes before your scheduled appointment. If you cannot keep your appointment, please call the practice or specialty service where you had the appointment to reschedule. Find the location of the outpatient practice or specialty service you are visiting by clicking the name of the practice or service at the left.
