Located on the main campus of Mount Sinai Health System in New York City, and as part of a nationally ranked hospital, the IMA offers comprehensive, personalized primary care and specialty services for adults in an outpatient setting. We specialize in caring for patients with hypertension (high blood pressure), diabetes type 1 and type 2, asthma, chronic obstructive pulmonary disease (COPD), cardiovascular disease, high cholesterol (hyperlipidemia), and organ and transplant patients.

We welcome patients from diverse cultural and religious backgrounds and offer care that is both culturally sensitive and holistic. Utilizing a multidisciplinary team approach, we work closely with our colleagues in surgical, medical, psychiatric, and rehabilitation services at Mount Sinai. In addition to the outstanding primary care provided by our highly experienced staff, we are able to quickly and conveniently make referrals and coordinate care with a vast array of outstanding specialty services at Mount Sinai.

IMA programs include:

  • Preventing Admissions Care Team (PACT) – PACT is a multidisciplinary team within IMA that provides intensive and comprehensive primary care to patients with multiple chronic conditions or hospital admissions. For more information, call: 212-824-PACT.
  • Heart Failure Education and Rehabilitation Training Program (HEART) – HEART is a nurse-led program within IMA that aims to improve qualifying patients’ ability to manage their cardiac health. For more information, contact: Elizabeth Vicente, RN, at: 212-824-7509.
  • Creating Opportunities to Achieve Control of Hypertension Program (COACH) – A nurse-managed program, COACH provides personalized education on the causes and consequences of high blood pressure, in addition to counseling on the importance of lifestyle changes and a home blood pressure monitor.
