For Your Visit
The Mount Sinai Hospital Division of Surgical Oncology has developed a list of instructions to help make the most of your visit with us.
Please plan to arrive at our practice 20 minutes before your scheduled appointment time to complete the registration process.
Please be sure to bring your picture identification and insurance cards with you to your appointment. Our office will handle the verification of your insurance.
When scheduling your appointment, please include an email address, if possible. We will email you a welcome packet that includes a short biography on your physician, a list of parking accommodations, and a reminder of our address and your appointment date and time.
We want to ensure that our physicians have the information they need to provide you with the highest quality care and therefore request that you have any medical records or test results from other physicians sent over to our practice one week before your appointment. These records may be faxed to our practice at 212-241-1572. We will scan your information into our electronic medical records so that it will be available to all care providers at the Mount Sinai Health System.
Examples of records we will need from you are:
- Your most recent lab results, pathology reports, radiology results and prior operative reports, as applies in your case.
- Our physicians will carefully review your original radiology images; therefore we request that you bring a copy of your radiology files on a CD, in addition to the written radiology report.
- If possible, pathology slides should also be brought to your appointment. Our colleagues in the department of pathology will carefully review your slides and can often provide additional information when needed.
- We understand that it may not always be possible to send records in ahead of time. If this is the case, please bring whatever information you have with you to your first appointment.
