Transition to Fitness Program

The Transition to Fitness Program at Mount Sinai provides participants an opportunity to improve cardiovascular health, muscle strength and flexibility, as well as their sense of well-being, through a personalized exercise program designed by a physical therapist in a motivating and supervised environment. The long-term goal of the program is for clients to learn to maintain a training program in their own community.

Potential participants must obtain medical clearance from a physician and a written referral. A physical therapist determines the client's fitness profile and abilities and tailors an exercise program to meet specific fitness goals.

Any restrictions or limitations are determined and the program can be modified appropriately. Each session is supervised to ensure safe and proper technique, allow for advancement of the individual's exercise program and assist with weight set-up and spotting.
