Getting Ready for Your Visit
Please plan to arrive at our practice fifteen minutes before your appointment time to complete the registration process.
If your insurance requires a referral, please be sure to bring it with you to your appointment. Also, please bring your insurance card(s).
To ensure that your physician has the information he or she needs to provide you with the highest quality care, please arrange to have any medical records or test results from other physicians sent over to the practice at least one week before your appointment.
Medical records and test results may be faxed to the practice in advance of your appointment.
And help us help you. Please remember to bring your glucose monitor to every appointment so we can download your numbers and analyze them during your appointment.
Tired of waiting in line and filling out tedious paperwork? Print out our forms (see forms in right column) and do it ahead of time!
If you have a glucose meter, please make sure the time and date are correct and bring it with you to your visit. Our team can download your glucose meter readings and analyze blood glucose (blood sugar) patterns.
Please fax your latest laboratory reports (or ask your physician to fax them) to (212) 423-0508 at least one day before your visit.
Remember that, since diabetes can affect the nerves and predispose you to foot ulcers, your doctor will probably ask to examine your feet. Plan accordingly and wear shoes that can be taken off easily. Please avoid wearing pantyhose.
