Patients Notified of Error in Disposal of Files
The Department of Preventive Medicine of the Mount Sinai Health System notified 1,586 patients on October 4, 2013 that due to an error by a third party cleaning service, confidential files were inadvertently included with regular trash on August 6, 2013.
The department expressed regret and advised the individuals that it is unlikely the files with their names (and in some cases date of birth, social security number and clinical information), were accessed, since the materials were recovered and secured the same day. The original intent had been to securely dispose of the files.
Patients with questions, or any evidence of actual identity theft, were advised to contact Mount Sinai at 1-888-702-0630. Mount Sinai offered to cover the cost of credit monitoring for one year.
To ensure that there is no recurrence, processes are being reviewed and revised, and the matter has been reported to affected individuals as well as the appropriate state and/or federal agencies as required by applicable law.
