Mount Sinai – Sema4 Health Discovery Initiative Patient Opt Out Registry
Mount Sinai strives to provide each patient the high-quality care they need, and the respect and privacy they deserve.
Mount Sinai has a longstanding relationship with Sema4, a patient-centered health intelligence company that was started in 2011 as an initiative within Mount Sinai. This collaboration is focused on improving the diagnosis, treatment, and prevention of disease. The new Mount Sinai – Sema4 Health Discovery Initiative (the “Initiative”) hopes to further improve the health of patients and clinical care in cancer, reproductive health, and other conditions and diseases. This Initiative will help Mount Sinai clinicians to better understand how each patient may respond to a specific treatment, or how likely they may be to develop a future condition based on their unique genetic, molecular and personal characteristics. Your health data, along with that of others, may also contribute to the development of new treatment options for you and future patients.
Although these advances are very important, Mount Sinai understands that patient care is about more than just science -- it’s also about the respect, privacy, and personalized attention that we provide each of our patients and their loved ones. We use state-of-the-art cyber security to safeguard patient data, and all data that is retained by Sema4 for its use through this Initiative is de-identified in accordance with HIPAA standards, which means that personally identifiable information, such as a patient’s name or social security number, will be removed.
Mount Sinai understands that you may choose not to participate. If you decide to register to opt out, we will not share your records as part of this Initiative. Please complete this form to opt out.
