The highly trained pediatrics team at Mount Sinai Doctors Faculty Practice Associates provides comprehensive pediatric care to our patients.

Our office provides care to your child from birth through age 18. We are committed to providing a full range of pediatric services—including routine well-child/preventative visits, complex care for children with multiple medical issues, sick care, guidance on development and nutrition, lactation, and parenting tips and resources.

We pride ourselves on getting to know all our patients and their families, allowing for individualized attention and personalized care. Our team works in association with the pediatric experts throughout the Mount Sinai Health System, to bring the most advanced and compassionate care to our community. Our goal is to involve you in decisions about your child’s health care to develop a stronger relationship with you and your family.

Office Policies

Camp, School, and Sport Physicals

We are happy to provide school and camp forms when requested. Please review the following information:

  • Forms can be completed only if your child has had a well visit within the last 12 months. If it has been more than 12 months, please call our office at 212-241-4242 to schedule an appointment for a Well Child Visit referenced above.
  • There is a $10 dollar administrative fee for all camp and school forms to be completed within 10 business days.
  • There is a $20 dollar administrative fee for expedited forms. Please allow two business days to be completed.

  • When submitting the form, please complete all the sections designated for the parent/guardian. Please note on your request if there have been changes in the medical history since the last visit and if a medication form is needed. Failure to complete all pertinent parts of the form will delay its processing.

Late arrival
In order to allow all patients to be seen at their scheduled time, we strictly enforce the late arrival policy. Please arrive 5-10 minutes prior to your scheduled appointment to allow time for elevator, check in process, etc. If you arrive 15 minutes or more after your scheduled appointment, we will consider you a “no show” and your doctor may not be able to see you.

Cancellations
If you need to cancel an appointment, please do so at least 24 hours in advance. You may incur a charge if you cancel an appointment without 24-hour notice.

Telephone/My Mount Sinai App/After hour encounters
When medical advice is given over the phone or via My Mount Sinai app, the provider is required to create an encounter for the purposes of documentation and billing. Depending on your insurance plan, these encounters may incur a charge or copay. The charge is determined by each plan and carrier, not by our providers nor our office. We encourage you to call your insurance to verify if these types of encounters will be covered.

Vaccinations
We follow the immunization schedule recommended by the Center for Disease Control (CDC) and the Prevention Advisory Committee for Immunization Practices (ACIP). We will make every effort to accommodate your family needs and concerns regarding vaccines, but it is our office policy to have all of our patients vaccinated by two years old as per the following schedule.
