Programs for Physicians and Patients

The hepatologists of Mount Sinai Health System have joined forces with our internists to develop an innovative program. Through this collaboration, you can receive treatment for your hepatitis C within a special primary care clinic setting. This comprehensive team approach to your care ensures that you receive supportive services from diagnosis through treatment.

Support Groups for Patients

Mount Sinai offers regularly scheduled support groups for hepatitis C patients:

  • The Mount Sinai Division of Liver Diseases offers a monthly support group for patients and those affected by hepatitis C. This group meets on the third Wednesday of each month and is open to all. Refreshments offered. No registration required.

  • The Mount Sinai Division of General Internal Medicine offers a monthly support group for patients and those affected by hepatitis C. This group meets on the second Tuesday of each month and is open to all. Refreshments provided. No registration required.

  • The Mount Sinai AIDS Center: offers a monthly support group for patients and those affected by both HIV and hepatitis C. This group meets one Monday of each month from noon -1 pm. Open to all. No registration required.

    The group is led by Jeffrey Weiss, PhD.

Hepatitis C Clinical and Research Program

The goal of our Hepatitis C Clinical and Research Program is to help identify those who have chronic hepatitis C. The Program also provides the latest and most effective treatments, serving the community by:

  • Conducting hepatitis C virus (HCV) antibody testing
  • Following up with HCV confirmation testing
  • Providing education about hepatitis C

This program is also known as our Hepatitis C Education and Liver Screening (HEALS) program. We evaluate and treat chronic hepatitis C infection through primary care providers based in the community. Our program uses a multidisciplinary team model—offering mental health, social work, and nutrition services for all patients.

Through the New York State Department of Health HepCAP program, we are able to provide you with medical care if you do not have health insurance. The program is open to all, including homeless and undocumented hepatitis C patients. We hold a monthly HepCAP group meeting open on the first Wednesday of every month from 6 - 7:30 pm.

Education for Health Care Providers

The Hepatitis C Clinical and Research Program provides education about chronic hepatitis C infection to Mount Sinai medical staff, including residents, medical students, and pharmacy students. The program also educates our peers throughout the community to help ensure that health care providers are able to meet the goals of the hepatitis C program. For patients receiving treatment from health care providers in the community, we provide education about hepatitis C.

In addition, our team researches the delivery of hepatitis C health care services and develops innovative tools. We review the process patients use to get appropriate care and treatment following a positive screening test. We also participate in efforts to help health care providers improve treatment outcomes through: 

For Referring Physicians

Collaborating and communicating with referring physicians in a timely and efficient manner is a top priority. Mount Sinai hepatologists understand the needs of referring physicians. We know that it is important for you to remain involved in each patient’s treatment plan as much as possible. As the referring provider, you have the most insight into your patients' medical history, and we highly value your input. Keeping open the lines of communication contributes to the consistent quality of your patient's care.

The process for referring patients is:

  • Have your patient call a Mount Sinai liver specialist at one of our locations to make an appointment.
  • Fill out the one-page Referring Provider Checklist, and fax it to 212-241-4465.
  • Provide your patient with his or her medical records to bring to the initial visit; or, if you prefer, you can fax your patient’s medical records to Mount Sinai at 212-241-4465.
