Mount Sinai Health System and Livongo Health Launch Innovative Effort to Better Manage Diabetes

  • New York, NY
  • (October 26, 2015)

The Mount Sinai Health System and Livongo Health™, a consumer digital health company, today announced the launch of a comprehensive diabetes management program in the New York market.  The program, already available to Mount Sinai employees, combines the Livongo for Diabetes solution with the clinical capabilities of the Mount Sinai Clinical Diabetes Institute and the Mount Sinai’s Diabetes and Cardiovascular Alliance to offer a comprehensive health management solution to patients with diabetes.  The program will be expanded to broader populations across the New York market through Mount Sinai’s network of physicians and affiliated providers.

"With several hundred thousand individuals covered under our value-based care arrangements, and that number growing rapidly, we are shifting our care models accordingly, and Livongo will play a key role in supporting our efforts to effectively manage diabetes—a chronic health condition that affects more than 10 percent of New Yorkers,” said Niyum Gandhi, Executive Vice President and Chief Population Health Officer, Mount Sinai Health System. “Mount Sinai remains committed to transforming healthcare in New York, and we are proud to collaborate with innovative companies such as Livongo to help us meet the goals of our health system and the community.”

The program provides an end-to-end ecosystem that empowers people with chronic conditions, including diabetes, to live better. It starts with a smart data analytics platform that captures information from a variety of sources including Livongo’s connected glucose meters, personal devices, electronic health records, and medication records.  Relevant and timely insights and alerts are then provided in real-time to the person with diabetes, their network of family and friends, and their Mount Sinai care team.

"We are excited to build on the great work Mount Sinai is doing in diabetes management today. With our connected technology, real-time data, and predictive analytics, we are partnering to bring a robust program to empower thousands of Mount Sinai  employees, dependents, and the health consumers Mount Sinai serves who are  challenged with managing diabetes to live better," said Glen Tullman, Chief Executive Officer of Livongo Health.
