Coordinated care involving doctors, nurse practitioners (NPs), physician assistants (PAs), registered nurses (RNs), and other professionals and providers working together to treat and monitor your health during and following an illness or injury
Primary and palliative care for complex patients who have difficulty accessing medical care
Home health care for eligible patients who need support between medical visits
