The Mount Sinai at Home clinical programs offer medical services for patients in the comfort of their own homes. Some of these patients might otherwise need to be in the hospital, emergency department, doctor’s office, or a rehabilitation facility. The Mount Sinai at Home programs can arrange for you to receive care in your home, such as:

  • Coordinated care involving doctors, nurse practitioners (NPs), physician assistants (PAs), registered nurses (RNs), and other professionals and providers working together to treat and monitor your health during and following an illness or injury
  • Primary and palliative care for complex patients who have difficulty accessing medical care
  • Home health care for eligible patients who need support between medical visits
