Accountable Care Organization
The Mount Sinai Health System has been selected to participate in the Medicare Shared Savings Program (Shared Savings Program) Accountable Care Organization (ACO), a health care delivery model sponsored by the Centers for Medicare and Medicaid Services (CMS).
The Mount Sinai Health System’s participation in an Accountable Care Organization (ACO) is an opportunity to integrate our innovative programs, pool resources, and develop new methods for streamlining care across a range of services — from chronic disease management, to specialty care and complex procedures, to promoting prevention and wellness.
The Mount Sinai Health System has one ACO program.
If your doctor chooses to participate in an ACO, you will be notified. This notification might be a letter, written information provided to you when you see your doctor, a sign posted in a hospital, or it might be a conversation with your doctor during the next visit. If your doctor participates in an ACO, you still will have the option to see any health care provider who accepts Medicare. To learn more about ACO, visit our Frequently Asked Questions (FAQ) page.

Our specially trained care coordinators and other health care providers, will work together to streamline communication and integrate our services to better serve our patients.
