Social workers are integral members of the team at Mount Sinai Medical Center’s Recanati/Miller Transplantation Institute for liver, kidney, heart, lung, and small bowel transplantation. They assess patients’ psychosocial suitability for transplant, assist patients in becoming an appropriate transplant candidate, and provide support to patients and families throughout the transplant process insuring continuity of care. Social workers assess patients’ support networks, financial stability, health care coverage, alcohol and substance abuse history, and any history of psychiatric illness; and make appropriate referrals to outside agencies as needed. After transplantation, social workers provide ongoing supportive counseling for patients and their families. Group therapy is offered to all transplant patients. Crisis intervention and bereavement therapy are provided as needed.
