Social Worker Program Specialist

The Social Work Program Coordinator is a new position. It reports to the Director of Care Coordination and its primary role is to provide oversight to the Queens primary care practices to ensure alignment with the Accountable Care Organization (ACO), Health Home (HH) and Patient Centered Medical Home (PCMH) outcomes and provide supervision to the care coordination staff in these practices. This position will collaborate with practice administrators, medical providers and perhaps staff at neighboring hospitals where these patients visit the ED or are admitted to establish systems and processes. This position will be part of the care coordination leadership team and attend regular meetings with other program coordinators covering other Mount Sinai Health System sites.
New York State LCSW required
Ability to develop and coordinate highly regulated service delivery systems.
Exceptional collaborative, organizational, communication skills to meet the challenges of complicated, evolving programs.
Exceptional skills in engaging patients and families and in assessing and coordinating resources within families and hospital and community networks for effective management of patient care.
Supervision experience of diverse staff preferred.
Hospital experience and experience with care coordination for a high-need Medicare (Fee for Service), Medicaid (Fee for Service and Managed) and the Medicare/Medicaid population preferred.
Strong interest in being part of an innovative initiative to improve the quality and cost of care.
Computer proficiency including ability to run/analyze reports in various data systems.
Willingness to travel regularly among the Queens primary care practices to provide oversight/supervision.
