CLINICAL MARKETING AND OUTREACH ADMISSIONS COORD

The Clinical Marketer/Outreach Coordinator assists the Admissions Supervisor in prioritizing referrals from both internal and external candidates for acute rehabilitation. This person also provides key communication between the referring care team and the receiving care team so that continuity of care is achieved.
1.Assists in admissions decisions, internal and external, that support compliance with the 75% rule, the 3 hour minimal programming need. and logistics of room and care needs
2.Ensures that financial clearance is obtained and that information is entered accurately and timely in hospital software applications
3.Aggressively works the screening function to insure that the census remains at or above 94% with appropriate patients.
4.Communicates critical and timely admission information to physicians, involved rehab leadership, and Finance
5.Assists in the retreival, delivery and record keeping relating to dishcharged medical records
6.Assists in the coordination of admissions efforts, both internal and external, integrating information from the field nurses and consult MDs
7.Attend management meetings in coordination w. admissions supervisor
8.Maintain internal referral list
9.Assists in the assignment of consult MDs/RNs
10.Communicate w/ in-house referrers re: status of pt appropriateness
11.Review all external records faxed to office and respond and coordinates decision and recommendation with admissions supervisor
12.Liaison w/ insurance companies, Medicaid office to obtain appropriate prior authorization disseminate financial information to appropriate hospital depts.
13.Interact w/ RN managers, social workers and case managers for planned admissions/discharges, and confirm discharges each morning
14.Attend hospital Bed Board meeting daily to coordinate decanting of pts into rehab.
15.Maintain computer records, including all demographics, clinical info and insurance information.
16.Tour prospective pts/family members and provide appropriate written information and resources. Answer questions and refer to other clinical personnel as needed.
17.Coordinate w/ hospital administration re: "special cases."
18.Maintain communication w/ floor personnel (RNs and MDs) re: admissions, including any pertinent medical information, special needs, etc.
19.Assume management duties as designated by admissions supervisor
20.Document all consults in rehab database
21.Initiate insurance precerts, as needed, and coordinate same w/ office
Performs other related duties
Masters Degree required.
At least 2 years experience in acute rehabilitation setting
License as required by clinical expertise (NYS PT, OT, SLP or RN)
