Job Description:


The Quality Assessment Coordinators are responsible for retrieving and collecting data relative to the quality functions of the Departments of Medicine and Surgery, as well as for reporting to outside regulatory agencies.

Essential Responsibilities

Assists the Chief of Service in drafting the annual PI plan for the department and selecting the quality indicators therein.
Evaluates quality improvement project performance indicators through medical records data abstraction, tracking and trending patient outcomes and physician practice patterns.
Evaluates documented care rendered through medical records data abstraction for reporting to regulatory agencies.
Coordinates the ongoing statutorily mandated Peer Chart Review (mortality, readmission, Department of Health case review, etc.) using pre-established criteria.
Schedules, coordinates, create the agenda, and assist the Chairman at the QA Committee meetings.
Analyze reports and identifies trends and statistical significance in metrics that will assist with focused as well as organizational process improvement.
Assists the Chief of Service in the development, revision, maintenance, and implementation of medical guidelines, policies and procedures, protocols, clinical pathways, and algorithms in accordance with established and approved standards of medical practice.
Maintains and distributes updates of documentation for the Departmental Policy and Procedure manual.
Participates in performance improvement task forces as assigned and participates in Hospital-wide multidisciplinary performance improvement activities.
Coordinates the data entry process into the QA Departments information system.
Assists executive staff with hospital-wide performance improvement efforts including regulatory survey preparation.
Performs other duties as deemed appropriate and assigned by the Director, and consistent with the mission of the hospital to deliver quality patient care.

Job Qualifications:

Education & Licensure:
Registered Professional Nurse licensed to practice in the State of New York
Bachelor's degree from an accredited college or university.

Training & Experience:
Three to five years clinical nursing experience in an acute care facility
Experience in Quality Assessment preferred
Ability to lead, supervise, and coordinate performance improvement activities
Working knowledge of regulatory and insurance requirements
Experience with medical records data abstraction, statistical analysis, and reporting
Knowledge and ability to use computer
Good oral and writing skills
Proficiency in other languages a plus.
