Preparing for Your Visit
Mount Sinai Doctors-Brooklyn Heights is on the 17th and 18th floors of the One Pierrepont Plaza building at 300 Cadman Plaza West. The building is located between Pierrepont and Tillary streets.
Several subway lines stop within two blocks of the office. The 2, 3, 4, 5, and R trains go to the Court Street–Borough Hall stop. A few blocks further is the Jay Street station, with the F, A, and C trains.
Arrive on Time
Please arrive 15 minutes before your scheduled appointment. If you are running more than 15 minutes behind, call us and our front desk will reschedule you to the next available appointment slot.

We encourage all patients to use MyMountSinai. This is your online gateway to Mount Sinai physicians and services and to all your medical records. The app enables you to take care of your health care needs on the go and communicate easily with your care team.
With MyMountSinai, you can:

Lab
As of Monday, October 2, 2023, we will no longer have an onsite lab. Here is what that means for our patients:
All bloodwork and labs must be performed during a scheduled in person visit or at an outside lab at a later date.
When you have a scheduled in person visit with us – and your doctor orders labs that can be completed that day – our clinical team will complete these as part of your visit.
For video visits, or if your provider orders labs to be performed on a different day, the order will be placed electronically and can be completed at any New York LabCorp location. The results will be sent to your MyMountSinai portal.
Radiology Procedures
All patients requiring diagnostic mammograms and ultrasound biopsies can be seen at our Mount Sinai Doctors-Chelsea or Mount Sinai-Union Square locations. You may continue to book screening mammograms and ultrasounds at Brooklyn Heights.

Getting your medical records can be complicated because of stringent privacy laws. Here are answers to the most commonly asked questions:
How do I get a copy of my medical records?
The fastest and easiest way to obtain a copy of your medical records at Mount Sinai Doctors-Brooklyn Heights is to contact our medical admin team by emailing bhforms@mountsinai.org and requesting a form for school, camp, or work.
How long does it take to get my records?
It usually takes two to three business days for us to prepare your records. You can get them by email, mail, or in person.
Can I pick up a copy of my records?
Yes. Email bhforms@mountsinai.org two or three days ahead of your appointment.
How do I have copies of my medical records sent to my provider at Mount Sinai Doctors-Brooklyn Heights from another doctor’s office?
You can complete a "Patient Authorization for the Release of Medical Information to Mount Sinai" form and send it to the provider who has your medical records.
