What to Bring to Your Appointment

• Prepare a list of your medical problems & health concerns that you want to discuss with your health care provider
• Bring your insurance information/identification
• Bring with you a list of your medications
• Please arrive 20 minutes prior to your scheduled appointment time for your first/new visit to the practice. This will allow the practice time to obtain insurance & other important patient information.
• Please arrive on time for all your appointments
• Ask questions if you do not understand something your health care provider told you during your visit
• Follow the instructions provided by your health care provider and make sure you arrange for follow-up appointments as instructed
