Patient Services

In addition to coordinating care with a range of health professionals in the hospital and home setting, the Visiting Doctors Program routinely works closely with home care, nursing, community, and hospice agencies.

Services provided to homebound adults include:

  • Preventive health care
  • Diagnosis and treatment of medical conditions
  • Prescription of medications, with an emphasis on safety and affordability
  • Referral for rehabilitation
  • Assistance with practical needs, such as wheelchairs
  • Support to family members and other caregivers
  • Treatment of pain and other symptoms

Patients in need of our services frequently have numerous conditions that require care from home health aids, visiting nurses, community agencies, hospice and others. Additionally even with the best of preventive care, these individuals require periodic hospitalizations. Our physicians and staff work with all the professionals providing care to our patients in their homes and in the hospital to ensure coordination.

Our nurses and social workers work with the physicians, patients and caregivers to meet the needs and wishes of patients and their families.

Providers also have extensive experience working with patients who are at the end of life, Sensitive to the needs and desires of all involved, providers work together with the patient and his or her family members to make sure care is delivered in a manner consistent with the patient’s wishes.
